I would describe my Life Coach Ela as knowledgeable, wise, attentive, good listener, approachable, organized and God centered. Council me on ways to do what I know I need to do but do not always want to do. Most significant change I noticed is the fact that I thought I knew enough about healthy living but there is so much more and it is not boring! Awareness of different healthy yummy foods to purchase and how to prepare them. Ela provided and showed me alternative healthy snacks that met my junk food desires i.e a homemade healthy cupcake which was delicious, better coffee where no sugar needed and more. The biggest tangible I am conscientious of Ela’s shared wisdom/voice in my head when I make decisions about buying and using junk food. The SLP is responsible for teaching and supporting the Individual’s life choices, hopes and dreams, and daily documentation of that teaching. The daily home routine should be based on the Individuals choice but structured as determined by the Individual and ISP team which is helpful to the Individual supported in learning to function independently in a home and community setting. SLPs have basic home support and related responsibilities, which include providing/supporting nutritional meals providing transportation to all appointments, to and from day services and recreational activities providing/supporting access to community activities, cultural and religious services, if appropriate, and providing up to 24-hour supervision and supports as outline in the individuals Individual Support Plan (ISP). is to build a community strong in family values Life Choices, because every choice affects a future Ruling year info. The primary responsibility of an SLP is to maintain a safe and healthy home environment for the individual supported. Integrated Life Choices is seeking Shared Living Providers (SLP) to provide residential support in the SLP's home for individuals with developmental disabilities.
